Trump’s DOGE Initiative Shreds USAID, Dooming Millions to Death
On Inauguration Day, Trump signed an executive order halting all foreign aid, unleashing Elon Musk’s Department of Government Efficiency (DOGE) to gut USAID. Despite public lies, critical contracts for malaria, tuberculosis, and other lifesaving programs were terminated, threatening 2.6 million additional deaths annually. This brutal dismantling exposes the administration’s callous disregard for global health and national security.
On January 20, 2021, President Donald Trump signed an executive order freezing all foreign assistance, setting in motion a ruthless campaign to destroy the U.S. Agency for International Development (USAID). At the helm of this assault was Elon Musk’s newly minted Department of Government Efficiency, or DOGE, a shadowy quasi-governmental body Musk used as a wrecking ball against USAID.
Musk, evidently clueless about USAID’s mission, openly declared his intent to feed the agency “into the wood chipper.” His ignorance and disdain were matched by the administration’s brazen dishonesty. Standing beside Trump in the Oval Office, Musk lied to the press, claiming that lifesaving programs to combat deadly diseases were continuing as usual. They were not.
Behind closed doors, USAID’s funding was frozen, contracts terminated, and its workforce decimated. The agency’s name was even removed from its headquarters as a symbolic gesture of erasure. On February 26, whistleblowers revealed that flagship contracts for malaria, tuberculosis, and maternal and child health programs—cornerstones of global health—were abruptly canceled. These diseases are among the world’s top killers, particularly of children under five.
The carnage extended beyond these three critical contracts. Nearly every global health contract, including those for HIV, polio, neglected tropical diseases, family planning, and nutrition, received termination notices. The consequences are catastrophic.
Internal memos from USAID warned the administration of the deadly fallout. One memo, titled “Risks to U.S. National Security and Public Health,” presented modeling that predicted 2.6 million additional deaths per year due to the funding cuts. The analysis detailed how these program shutdowns would accelerate disease spread, destabilize fragile regions, and increase security risks—direct threats to American interests.
This assault on USAID was just the first step in a four-part blueprint for authoritarian dismantling: fabricate false stories to justify cuts, cripple IT and financial systems to stall operations, terminate contracts to prevent recovery, and demoralize staff to eliminate expertise.
Medical professionals and health advocates have condemned the move. Dr. Don McCanne, a physician and advocate for comprehensive healthcare, described the experience as heartbreaking, noting that some of the nation’s most powerful figures seemed indifferent to the value of public health. Dr. Jim Kahn, an expert in global health economics, emphasized that USAID’s programs were not only life-saving but incredibly cost-effective, generating goodwill worldwide.
The destruction of USAID under the DOGE initiative is a stark preview of the Trump administration’s broader pattern of corruption, incompetence, and authoritarian overreach. It is a brutal reminder that when government agencies tasked with protecting public welfare are sacrificed for political theater and ego, millions of lives hang in the balance.
